Bias Incident Reporting Form

Use this form if you have been the target of a bias incident or have witnessed a bias incident. A bias incident is an act of bigotry, harassment, or intimidation that is motivated in whole or in part by bias based on an individual’s or group’s actual or perceived race, color, creed, religion, national origin, gender, age, marital status, disability, public assistance status, veteran status, sexual orientation, gender identity, or gender expression.

 

CARE Team Referral Form

Use this form to report someone who may be experiencing distress with the support and access to College and community resources needed to be successful.

 

Discrimination/Harassment Complaint Form

Use this form to report an incident of discrimination or harassment at Rochester Community and Technical College.

 

Incident Reporting Form

Use this form to report any non-academic incident you witness on campus. If there is an immediate threat, call 911 immediately.
